Charpter 7 and my mortgages

hi, i never really fell behind on my mortgages (i have 2). i have been late on the second one only a week for the past few months but it's always been paid that month.
but i have a couple question:

what does it mean to "reaffirm", would this show on the bankruptcy papers??

i would like to have a loan modification on my second mortgage (it's 11%), do i just work with my current leader??

does the same rules apply, if i miss 3 payments then foreclosure starts? or can they start it sooner?
