In my BK Chap 7, which was discharged, my mortgage was included, but noticed the reaffirm box was checked off, I never received anything from my bank as far as reaffirmation....so does this mean that it is reaffirm?

You need to consult an expert about that and seek for an advise about your problem. Aside from that, you have to make an appointment to your bank and clarify things, it is not wrong to inquire since you are their client.
