If I have any other debt that I'm not aware of, are they required to notify me before reporting to my credit. I think I have everything cleared up, but I just paid a gym membership from years ago that wasn’t on my credit, but I forgot I even owed them money. Luckily I remembered this and paid. But if there is anything else I've forgotten about can it just show up, or will i be given a chance to pay it first?

If you don't pay any of your debts, then the creditor may directly report it to the credit bureaus. If your account has gone to the collection agency, then they may contact you for the payments and in the same time they may report it to credit bureaus.

As far as I know, the creditors can directly report the negative items to the credit bureaus. I don't think it is mandatory for the creditors to inform you before reporting the negative item to the credit bureaus.

No. As soon as you sign up for credit, it's reported to the credit bureau's, and a "tradeline" is added to your credit report, reflecting your agreement. If you forget about it, your credit can get ruined, because the creditor will report it hasn't been paid every month.
