If you wish to change names on the title, you may do so using a grant deed or a quitclaim deed. The latter is mostly used if there is a debt against the property.

You cannot change the deed on your own. Your father needs to quitclaim you to transfer the property to you. So talk to him and see if he is willing to quitclaim to you. If the title is free from any kind of liens then you can use a grant deed.
BTW don't forget to notarize and register the deed in the County Recorder's office in your state.
