how do my wife and I go about adding my (the husband's) name to the deed, so that we both equal on the deed?

Is the property in your wife's name only?
In that case she has to sign the quit claim as the grantor giving you half of the interest in the property.
Do you have a mortgage? Then you have to talk to your lender before doing a quit claim.

Jenkin is right. If your wife is the sole owner of the property, then she can just quitclaim her half share of the property and add your name to the title. Make sure that you have the deed valid by notarizing and recording the deed in your county recorder office.
You should contact with an attorney who will help you to make the valid.
