It looks complicated to fill out..Does it have to be notarized in ny..My hub wants to add my name to the deed..mtg done in a few months..is there an easy way to do this..Also my name was supposed to be on deed in 93 when we purchased..on the cover sheet my name is typed in and on all the mail from attnys etc..and the contract I believe..there was an error somewhere and attny out of bus..how can I correct this..I have all checks,etc, I disabled now and on ssd in ny ...will this effect my disability allowance at all..

Yes, it will be better if you could notarize the deed in NY. You can visit a County recorder will all the documents that you have and ask him to rectify the deed. Or else you can ask the bank or the lender can offer you a form by filling which you can rectify it.

The banks also offer a form by filling which you can rectify the deed and add your name to it. You can speak to the bank executives or your agent and ask them to offer this form.
Yes, it is very important for a quitclaim deed to be notarized in that state where the property is. If the property is in NY, you should get it recorded there.

For my surprise my last name was wrong on the warranty deed and I just see this after years

You need not worry about the fact that your last name is wrong on the deed. What you can do is to get in touch with county clerk's office and ask them how to change the last name on the deed. However, I think you may be asked to prepare a quitclaim deed, transferring the title from you as grantor with the wrong last name to you as grantee with the correct last name. For that you need to prove and convince the notary public that both of you are the same person.
