I would like to add my sister onto the deed so that she can claim half of the homestead exception. She has been living in the house for the past 10 years. IS this the form that I need to use

You can use a quitclaim deed to add your sister on the deed. Is it a mortgaged property? If so then you will have to take permission from your lender before signing the quitclaim deed.
Generally to get qualify for the homestead exception, the homeowner must be at least 65 years of age, or permanently disabled. So is she at least 65 years of age or permanently disabled. But the criteria to qualify for homestead exception vary from state to state. You should consult with a tax assessor on this matter.

My property is mortgaged and this is what I was told to do by my mortgage co. In the state of Florida, everyone that lives here full time gets a homestead exemption. I had the exemption on the home until I moved and it transferred to my new home

For your sister to claim half the exemption, you can add her to the title using the quitclaim deed. Check out the Florida Quitclaim deed form available here. But do get it checked by an attorney for any modifications in drafting the deed as per your requirements .
Since you'll quitclaim your a part of your share of interest, there are chnaces of paying gift tax. But there are exemptions too. Know more on Gift Tax exemptions.
