Where can I get a blank form for the above that can be used in AZ? All the forms I have seen online so far are drafted for CA.

I found a sample grant deed for Arizona which I believe can be used for interspousal transfers at the following page:

However, it is always better to get a grant deed drafted by an attorney. A sample form might not be appropriate for your particular situation. Even if you use an online form, it is advisable that you get it reviewed by an attorney to make sure you are using the right form.
