you can use a quit claim deed to add your spouse's name to the title of the house.
to get a mortgage it is not necessary to have your spouse on the title. you can qualify on your own if you want. but if you want to have your spouse as co-borrower then it will be necessary to have her name on the title.

are you willing to add your spouse on the deed only or also on the mortgage. if you want to add him on the deed only then refinance may not be needed but if you want to your spouse on the mortgage then refinance is needed.
another way is novation but lender does not always agree for that. so first talk to the lender and see if he allows you for that.
