I just got home from the notary signing the permanent modification docs from BofA when I noticed that he misspelled my name. Should I request BofA send out a new package for signature or can I just use a scanned/printed copy of the blank document? There are bar codes on the backs of the documents and I don't want to risk it.

It will be better if you could request BofA to send out a new package to you wherein you can fill in your correct name and other required details.
