My husband would like to add myself and our 3 children as Joint Tenants with right of survival to the deed of a property he owns outright in Berwyn, IL. I checked with the Cook County Recorder of Deeds and was told to
file a Quit Claim form along with an Exempt Stamp from Berwyn. Is this
all that is necessary?

Call the clerk of the recorders office to ask about getting these forms. They may be available to print online. You also may need your last utility/water bill to show your occupancy. Also, you will have to pay the exemption tax stamp fee charged by Berwyn, and the recording fees, which are generally a flat rate for a certain number of pages and extra fees for additional pages.
