How do I get a deed in my name?

My husband bought a house a few months ago with the loan in his name as I had filed bankruptcy a few years ago. I gifted him a large amount of cash for the down payment and am making half the mortgage payment. I found out that the deed is in his name only even though we requested both at the signing. How do I get my name on the deed?
