My brother and I own a home together and he wishes to sell his 50% stake to me. Im not sure how to fill out the quit claim form for an equity buyout between brothers?
Also, IM not sure if if I use the buyout price for the documentary transfer tax fee of $0.55 cent per $500 or the full value of the home?

It is your brother who needs to sign a quitclaim deed in order to transfer the property to you. Your brother and you can contact an attorney and he will help you in drafting the deed as well as in filling it out.
