My grandmother is getting old in age and wants to sign over her home to me. She does not want to give up the home until she passes. She wants to remain with all responsibilities and once she passes she wants for me to take over. What would I need to do to ensure that it stays they way it is currently and then comes to me once she passes?

You can ask your grandmother to draft a will and declare you as the beneficiary of the will. Thus, after her death, you can probate the property and get it transferred in your name. You and your grandmother should contact a real estate attorney and he will help her in drafting the will.
