Hi faithopelove!
Welcome to forums!
You can consider both the lots as one and use one quitclaim deed or warranty deed. Your parents can then add your name as one of the grantees to the property and include the clause of Joint tenancy with right to survivorship in it.
Sample quitclaim deeds are available online but it is always better to take the help of an attorney and draft the deed from him. You will then have to notarize and record the deed in the county recorder's office. If you consult an attorney, you will have to pay the attorney fees and apart from that you will have to pay the fees for stamp docs recording of the deed.
Feel free to ask if you have further queries.
