I signed an interspousal transfer grant deed. My ex is ready to refinance. What must I do to ensure I receive my 50% equity in the home?

at the time we divorced, due to my wife having an affair, our home was paid for; free and clear . i signed an "interspousal transfer grant deed" stating when the grantee may choose to sell or refinance the property, i will receive my 50% value of the home. my ex has agreed to refinance so i can receive my value of the home. what steps or what forms must i complete to allow my ex to refinance the home and ensure that i receive my share of the home's equity?
