How to fill out a Qucikclaim Deed

I'm trying to take my boyfriend off my deed. When I purchase my home, I need his income to help my afford the home. We are no longer together and he wants his name off the deed. He is aware that his name is on the loan but he just wants his name off the deed. How should I fill out the quickclaim deed? Grantor would be john doe & jane doe. The grantee would be jane doe only? (Right)

You will have to ask your boyfriend to fill out the deed and transfer the property to you. This will make you the sole owner of the property and he won't have any further claims on that property.
