we( i and my husband) bought our furst house 5 years ago.
back then my credit was not good.
so i signed the quit claim form so that the house can be loaned on my husband name only.
now we try to put my name back to the title again.
what is the best way?
we still pay mortgage.
quit claim will do?
do we need to notarize or just sign and send to the court?
can we just go to court and do the form there?

Your husband needs to sign on the quitclaim deed and get it notarized by a notary public. Then the deed should be recorded at the County Recorder's office. Depending upon the state you are in, the deed may or may not be signed by you (grantee). Regarding the drafting of the quitclaim deed, you may contact an attorney to help you out.

we are in california. i signed the quit claim form in the escrow company 5 years ago. could it be better if i call a escrow company to get help?

You can call the escrow company and get the help. You can also take the help of an attorney and get the deed drafted by him/her and then sign it. You will have to notarize and record it at the county recorder's office.
