My wife and I are currently seperated with the plan to get a divorce. Both of our names are on the title and currently her name is on the loan. I have paid the loan since its beginning. I am refinancing the loan into my name alone and she has agreed to sign the house over to me. Since both of our names are on the title, where it asks for the grantor on the qc form do I put both of our names on that line since we are both on the title? also when it states the grantor releases all interest in the proprty for the sum of... what do i put there since it is a transfer without any monies being paid out?

You can put both the names as the grantor of the property and your name as the grantee to it. As far as the sum of money is concerned, you can put it as $1 or you can have a word with an attorney and take his opinion in this regard.
