How do i remove my name from a deed

i thought my name was removed from the deed, when my ex husband and i divorced many years ago.. today i find out through court records my name is still on the deed and i only had my name removed from the mortgage, which would make me not responsible for the loan. this all comes to light when trying to figure out if i am eligible for first time home buyer status. how do i remove my name from a deed? if its the quit claim deed, where do i go and how much does it cost..
