if a quit claim deed to remove an ex spouse is filed with the city you have the house in, does the title company inform the mortgage company of this change?
if so, does the mortgage company force the other spouse to refinance?

no...that would be quite unusual if it occurred.

I'm not aware of any such incident. However, once you record the deed in the public record, it's open for public viewing. In case the lender comes to know about this change, they may call the balance amount due and ask the other spouse to pay it off immediately, or they can also ask him/her to do a refinance.
