when two people have ownership of a deed and the one wants to sign off his interests, do both the grantors have to have there name on the form to transfer to new grantees?

As far as I know, you will have to inform the the co-owner of the property about the transfer of property to the new grantees. The co-owner will have to sign the deed in this regard.

No, only the person transferring their interest has to sign.
