My wife has passed, I want to remove her name from the mortgage. Other than the death certificate, is there any other documentation required? Also do you need an original death certificate, or will a copy suffice? If original is required please confirm that it will be returned.

you will have to refinance the mortgage in order to remove your deceased wife from the loan docs. you can carry the original death certificate when you meet your lender for refinance. if her name is on the property deed, then you can file an affidavit of heirship at the county recorder's office and get the property transferred in your name.
