Transfer mortgages between family members

Is it possible to transfer mortgages between family members without informing the lender?
Transfer of mortgage from one person to another means that the new owner of the mortgage will have to refinance the loan. As he or she refinances the loan, the lender will come to know about it.

I personally believe that it is not a good idea to do anything related to the mortgage without informing the lender.
